I don't know about your first question, but question 2 they talk about that scene in the commentary on the DVD. When Dante tosses the keys you see a flash. The flash is a burn on the film.
If you get the chance I highly recommend watching the DVD or laser disk commentary. That is some of the funniest shit I've heard.
