Depends on who owns the rights to the song. In most cases of small, independent labels, the bands own all the rights to the songs. All you need is written permission from the band. However, the higher up the musical food chain you go, the more likely the label owns the rights to the music, and you need permission from the label as well. Best bet is to get EVERYONE's permission you can.
Also, if you're looking for a major label band and are also looking for a major studio to pick you up, you can get what's called "festival rights", which gives you the right to use the song/piece at film festivals. It's usually a lot cheaper, but you CAN NOT release the movie theatrically or on video with these rights. This is why, if you're making a movie, get a lawyer that knows the ins and outs of entertainment law. Like insurance, it's worth the extra couple thou.
