Which is a site as riddled with errors as trivia facts, but look at it like this.
Had Kevin ever produced and made the movie, NAME. Jay's last name would have been Derris. As he didn't and went a different way it wasn't/isn't.
Therefore, taking into consideration most of Mewes' autographs were signed Jay "Phat Buds" Mewes. It is entirely possible that someone who contributed to the imdb took this to be the CHARACTER'S last name.
It is also entirely possible that the AP wire reporter that this story came from referred to the imdb.com for the character's name when writing the story.
Hence
Jay, the character you know and love became Jay Phat Buds....
Just as if Silent Bob had a last name, I believe Kevin once stated, it would be Blutarsky.
So there you have it. Jay Phat Buds and Silent Bob Blutarsky.
