as to the second time, uh, he's been captured and the remaining eye was given to the big boss man... where's the risk? maybe the guy whose job it is to erase people's access had the week (or a couple days anyhow) off. maybe they didn't care... human error... any number of answers, all of which add to the realism...
if, however, you want to find something to actually pick on... when he gets locked in his car he clearly kicked the door OFF the little car, but after it went over the edge he climbed right up on the glass that he had just kicked off.
i liked Minority Report the first time i saw it, but the second time i loved it...
