Just because state and federal agencies offer programs, by no means, do they cure all ills in the world. They operate very much like those looking to turn a profit. In order for these to stay afloat, they need enough funding to pay for the program and its staff. Otherwise it goes away.
Also there are many guidelines for acceptance, much like any HMO/PPO/Fee for Service types, and limitations to the coverage. Suprisingly enough these programs will "cap" coverage at a certain number. These fall along the "Not-For-Profit" lines. While they try to be hospitable, there are certain realities about what they truly offer.
