I was under the impression that the camera rig you're describing- I don't know what it's called exactly- was specifically constructed by the DP who shot REQUIEM FOR A DREAM and PI. Basically, it's a harness that attaches the camera TO the actor, aiming it at them, so the person in effect becomes his or her own "dolly". Martin Scorsese did a similar thing starting in MEAN STREETS, but in that case the actor was standing still on a dolly facing the camera and the dolly is moved and the actor feigns "walking" (or in the case of MEAN STREETS, dancing). Spike Lee used this effect in JUNGLE FEVER.
