: I was under the impression that the camera rig you're describing- I don't know what it's called exactly- was specifically constructed by the DP who shot REQUIEM FOR A DREAM and PI. Basically, it's a harness that attaches the camera TO the actor, aiming it at them, so the person in effect becomes his or her own "dolly". Martin Scorsese did a similar thing starting in MEAN STREETS, but in that case the actor was standing still on a dolly facing the camera and the dolly is moved and the actor feigns "walking" (or in the case of MEAN STREETS, dancing). Spike Lee used this effect in JUNGLE FEVER.

According to Aronofsky's commentary on REQUIEM FOR A DREAM the rig he used is called the SnoriCam (sp?) because it was developed by two brothers (The Snoris) he was friends with for their photography. Aronofsky and crew adapted it for a motion picture camera, including what was probably a more complicated rig for REQUIEM since the actors were supporting a 35mm camera.
I assume the rigs used in the other movies mention were about the same thing, but probably not "SnoriCams."
