OT: Some Reservior Dogs questions

I love the movie and can't wait for the 10th Anniversary DVD this june, but I have a few questions.

SPOILERS!!!!!!!!!!!!!!! (in case someone hasn't seen it)

1. In the end, where they all shoot eachother, Joe has a gun on Mr. Orange, Mr. White has a gun on Joe, and Nice Guy Eddie has a gun on Mr. White. But after they all fire, Nice Guy Eddie dies too. Who shot him?

2. Reservior Dogs is a cool title, but what the hell does it mean?
